I accept terms & conditions of this registration. I hereby consent that my artwork be displayed by South Bay Art Association, Bellport Village, and any members/representatives thereof to promote myself and the exhibit. Any sale of artwork is subject to 10% donation to South Bay Art Association.

By checking this box, if accepted, I agree to drop off and pick up my artwork on the dates & times provided by South Bay Art Association. If I am unable to make the dates, it is my responsibility (the artist) to arrange someone else to deliver or retrieve my work. If Artist picks up work prior to the end of the exhibit, without notifying SBAA, they will be denied registration into future shows.
